Can I require a vaccine the certificate doesn't list a due date for?
Yes. Your door policy is a list of required vaccines plus a maximum months-since-dose for each. A record with no date given is flagged for the desk to look at, not turned away.
Do owners have to download an app?
No. Everything is a web page — the upload form, their records portal and the pet card, which they can open or share from any browser.
What does it cost for a daycare with 200 dogs?
